To the extent possible utilize more fresh, not frozen, whole foods wither fewer ultra-processed ingredients incorporating fresh, in season fruit and vegetables.

The contract entity must serve meals on time, at appropriate temperatures, and be visually appealing and prepared in a manner so that the taste is enjoyable.

The contract entity must provide (when requested from participants) viable alternatives to stated menu meals (i.e., gluten-free, lactose intolerant, diabetic, cardiac, renal, etc.). Menus will be provided by Dietary Solutions.

The contract entity must provide disposable items needed for the meals being offered that meet acceptable quality standards for use by older adults.

Disposable items include flatware and napkins.

The contract entity is encouraged to offer a variety of engaging activities. Activities should include life enrichment opportunities with the goal of reducing social isolation. Contract entities should track social engagement opportunities which can be done in multiple ways i.e., on the monthly menu. The contract entity is encouraged to document a list of activities and the number of attendees. The contract entity is not required to submit this report to Aging Services with the monthly reimbursement report but is encouraged to keep a record on file at each site. A conversation about social engagement will be part of the site monitoring.

The contract entity must collaborate with Aging Services to conduct an annual satisfaction survey. On or about November 1st, Aging Services will generate a random list of individuals for each contract entity to survey. The list along with a standardized survey template (example Satisfaction Survey) will be sent to each contract entity. Contract entities must print the survey on letterhead and mail to each individual that was randomly selected by Aging Services. The survey will include instructions how on to return the survey to Aging Services along with an enclosed self-addressed stamped envelope(s) (Aging Services will provide the envelopes to each contract entity). This process allows the individual to remain anonymous and assures no retaliation. The survey will contain the following components.

Aging Services reserves the right to place contract entities into remediation status if a deficiency is found regarding a performance measure(s) and/or policy and procedures. The contract entity must create a remediation plan to remedy the issues identified in the contract monitoring process. The remediation plan will include corrective actions taken, a plan of future corrective actions, and a timeline to complete the plan.
